Free T3 and Free T4 are absolutely critical! TSH tells you what your brain is *asking* the thyroid to do, but Free T3 and T4 tell you what the thyroid is *actually producing* and what's *available* for your cells to use. Many symptoms persist even with "normal" TSH if these are off.

Reverse T3 (rT3) is another important one, especially if symptoms like fatigue and weight gain persist despite decent Free T3/T4. High rT3 can block active thyroid hormone from reaching cells. And don't forget thyroid antibodies (TPO and TgAb) to check for autoimmunity like Hashimoto's.

Nutrition is huge for thyroid. Ensuring adequate selenium, zinc, iodine, and tyrosine are key precursors and cofactors. Brazil nuts for selenium, seafood for iodine, etc.

And sleep deprivation increases thyroid-binding globulin (TBG), which binds up thyroid hormones, making less free hormone available to cells. Sleep really is foundational.
