Also, not warming up properly. Dynamic stretches and light cardio before lifting heavy makes a huge difference.

Under-recovery! Not getting enough rest between intense leg days, or pushing through pain. Sometimes, a deload week or active recovery is exactly what your knees need.

Lack of mobility work and neglecting the surrounding muscles like glutes and hamstrings. Weak glutes can lead to knee valgus (knees caving in) during squats, which is terrible for the joint. Incorporate hip abductor and adductor exercises.
